Update of U.S. Guidelines for Cholesterol Management

U.S. cardiologists have updated their official guidelines for managing cholesterol and preventing dyslipidemia, implementing a comprehensive revision that replaces the 2018 version. According to ScienceAlert, approximately one in four American adults has elevated low-density lipoprotein (LDL) cholesterol, a major risk factor for cardiovascular diseases, which remain the leading cause of death worldwide.

Dr. Roger Blumenthal, chair of the guidelines writing committee at Johns Hopkins University, noted that more than 80 percent of heart disease is preventable. The new approach advocates for earlier pharmacological intervention than was recommended a decade ago if lifestyle modifications alone prove insufficient.

Key changes include:

- Initiating cholesterol screening for children aged 9 to 11, or as early as age 2 if there is a family history of heart disease.

- Adopting a new risk assessment tool called PREVENT to calculate the 10-year and 30-year risk of cardiovascular disease for individuals aged 30 to 79.

- Establishing stricter LDL cholesterol targets: below 100 mg/dL for low-risk individuals, below 70 mg/dL for those at high risk, and below 55 mg/dL for patients with severe cardiovascular disease.

Dr. Pamela Morris of the Medical University of South Carolina highlighted that clinical trials have demonstrated clear benefits from lowering LDL cholesterol to levels lower than those recommended by previous guidelines. The new guidelines were published in the Journal of the American College of Cardiology.
