Millions of Preterm Births Linked to Air Pollution

In the first global study of its kind, researchers from the Barcelona Institute for Global Health estimated that continuous exposure to fine particulate matter (PM2.5) air pollution causes 5.8 million preterm births annually worldwide, accounting for 16 percent of all preterm births. The report, published in The Lancet Global Health, analyzed data from 204 studies over 20 years and found a clear causal relationship: every increase of 10 micrograms per cubic meter in PM2.5 concentration raises the risk of preterm birth by 12 percent.
As Dr. Mark Nieuwenhuijsen, one of the study’s contributors, explains, the mechanism of harm lies in the ability of nanoparticles to cross the placenta (which was previously considered a natural barrier) and reach the fetal bloodstream, triggering systemic inflammation that stimulates the release of hormones leading to early uterine contractions. The study confirms that the most affected regions are South Asia (India, Pakistan, and Bangladesh), where average PM2.5 levels reach 80 micrograms per cubic meter—eight times the World Health Organization’s safe limit.
Among the key findings: 52 percent of pollution-related preterm births occur in India alone. In China, anti-pollution policies implemented between 2013 and 2026 helped reduce preterm births linked to particulate matter by 28 percent. Even in Europe, where average PM2.5 levels stand at around 15 micrograms per cubic meter, pollution causes 200,000 preterm births annually.
The World Health Organization has urged governments to accelerate the transition to clean energy and to provide pregnant women in polluted areas with subsidized home air purifiers. It also recommended that doctors more rigorously monitor placental function in high-pollution areas. The report concluded that combating pollution is no longer just an environmental issue, but a matter of safeguarding the health of future generations.
